Covid-19 Disclosures

I do not exhibit any symptoms of Covid-19, including fever, soreness, fatigue, persistent cough and/or difficulty breathing.
If I experience any of the symptoms mentioned above in the two weeks prior to my session I will reschedule.
If I learn I was exposed to Covid-19, I will reschedule my appointment for after the recommended quarantine period (2 weeks).
I will notify Fadeaway if I am diagnosed with Covid-19 within two weeks of visiting the center.

Float Tank Accessability and Facility Safety

Float Tank Damage/Contamination:

If I physically damage the float tank or contaminate the float tank solution I understand I am responsible for the following costs: lost income due to tank downtime, Epsom salt solution replacement and various cleanup expenses. A minimum charge of $1000 applies.
Session Start Times/Late Show Policy:

Float session start times are on a fixed schedule. If you are late your time in the tank may need to be cut short so the clients scheduled after you are not impacted. NO REFUNDS for shortened sessions due to client tardiness. Please arrive 10-15 minutes prior to your scheduled start time. On your first visit some float time may be lost due to the new client walkthrough.
Late Cancellations/No Show Policy:

24 hour notice for cancellations is required. Failure to provide notice in a timely manner will result in a float being deducted from your account, or up to the full session cost being charged to your credit card.
Beginner Expectations - EXTREMELY IMPORTANT:
Floating is a life-improvement practice that requires commitment; much like meditation, a healthy diet, or an exercise regimen. Studies on floating show that consistent sessions are the key to obtaining the kind of profound, well-documented mental and physical benefits floating has to offer.
If you have only floated once, you don't really know what floating is like. In this ever-distracted society, reaching deeper levels of relaxation takes practice. Do not get discouraged if you don't feel like you "got anywhere" your first session. This is completely normal. Please give floating a real chance and commit to 2-3 sessions. The benefits are worth it.
Float Tank Accessibility
Clients must be physically capable of entering and exiting the floatation tank (which has a 20” lip that must be stepped over upon entry/exit) on their own, and without the presence of grab bars. Clients must be physically capable of lowering themselves into the solution without assistance, and also capable of rising from a recumbent position without assistance. For liability purposes Fadeaway employees are not able to assist clients with getting in and out of the float tank. The float tank solution is slick and clients must use the utmost care when entering/exiting the tank and while utilizing the float center facilities. Fadeaway Floatation Center is not responsible for any injury or death caused by slipping and/or falling.
Pregnancy and Floating
It is imperative that you discuss floatation therapy with your prenatal care provider before reserving a session. The earlier on in the pregnancy you begin your floating practice, the more comfortable you will be later on as the pregnancy progresses. Modified positions and floatation devices may be helpful. Please ask an associate for details.
Safety Agreement:
It is expressly agreed that use of facilities undertaken by me is at my own risk, and that Fadeaway Floatation Center shall not be liable for any claims for damages, injuries or death alleged to have been caused by any activity at the Float Center. By signing below I agree and understand the terms and conditions of this electronic waiver and attest to the accuracy and completeness of each response.
